North Bay Regional Center is a nonprofit organization dedicated to providing support and services to individuals with developmental disabilities in Napa, Sonoma, and Solano counties. We are seeking a highly skilled Social Services Case Manager to join our team.

Job Description:

The successful candidate will be responsible for managing the delivery of services to regional center clients, including those with intensive physical or emotional needs. This will involve ensuring that clients' Individual Program Plans and objectives are pursued, and that community resources are made available to support their goals.

Key Responsibilities:
  • Monitoring vendor services to ensure they are delivered in accordance with purchase of service authorizations
  • Evaluating the effectiveness of clients' Individual Program Plans and making recommendations for improvement
  • Conducting annual reassessments of clients' developmental status in conjunction with the interdisciplinary team
  • Assisting clients in accessing public and private resources to support their needs
Requirements:

To qualify for this role, you must have a Bachelor's degree in a field related to developmental disabilities, such as Social Work, Psychology, Counseling, Special Education, or Nursing. A Master's degree is preferred. You should also have at least 3 years of experience working with individuals with developmental disabilities, and bilingual language ability (Spanish to English) is highly desirable.

Estimated Salary:

Ranging from $65,000 to $85,000 per annum, depending on qualifications and experience.
