Job Description
We are seeking a skilled Physical Therapist to join our team as a Chronic Care Therapist Specialist. As a member of our multidisciplinary team, you will be responsible for planning, organizing, and delivering physical therapy programs that help clients recover and improve their mobility, relieve pain, increase strength, and prevent deformities. Your expertise will be utilized to conduct examinations, evaluations, and interventions for clients affected by injury or disease. You will also be responsible for documenting client information, establishing therapeutic plans, and modifying them as necessary. Additionally, you will educate clients and their family members about treatment plans and self-exercises to be continued at home. You will have the opportunity to work with individuals of all ages with varying diagnoses and disorders, utilizing your knowledge of current research to inform your treatment approaches.

Required Skills and Qualifications
To be successful in this role, you will need to possess a degree in physical therapy from an accredited institution and be licensed in the state(s) of practice. You must also have Basic Life Support (BLS) certification for healthcare providers. Other requirements include the ability to work in a fast-paced environment, flexibility with variable work schedules, and a commitment to ongoing education and training. If applicable, a valid driver's license may be required.
