We are seeking a dedicated and compassionate Pediatric Home Health Nurse to join our team at Thrive Skilled Pediatric Care LLC. As a nurse with Thrive, you will have the opportunity to make a meaningful difference in the lives of medically fragile children and their families.

About the Role

We are looking for a nurse who is passionate about providing high-quality, patient-centered care in the comfort of the child's own home. As a Pediatric Home Health Nurse with Thrive, you will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each child.

Responsibilities
  • Provide skilled nursing care to medically fragile children in their homes
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Conduct thorough assessments and evaluations to identify the child's needs and goals
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Document all care provided and communicate effectively with the child's family and healthcare team
Requirements

To be successful in this role, you will need:

  • A current, valid RN or LPN license to practice in the state of Kansas
  • Current CPR certification
  • Experience working with medically fragile children, preferably in a home health setting
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
