We are seeking a dedicated and compassionate Pediatric Home Health Nurse to join our team at Thrive Skilled Pediatric Care LLC. About the Role

We are looking for a nurse who is committed to delivering exceptional care and support to our clients. As a Pediatric Home Health Nurse, you will be responsible for providing one-on-one care to our patients, working closely with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Provide high-quality, patient-centered care to medically fragile children in their homes
  • Develop and implement individualized care plans in collaboration with our interdisciplinary team
  • Work closely with families to ensure seamless care transitions and support
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements
  • RN/LPN with a current valid license to practice as issued by the State Board of Nursing
  • Current CPR certification
