Salary

The estimated salary for this position is $65,000 - $90,000 per year, depending on experience and qualifications.

Job Description

As a Nurse Care Coordinator, you will work closely with our healthcare team to provide comprehensive care to our clients. Your primary responsibilities will include:

  • Conducting in-depth patient interviews and assessments
  • Developing and implementing individualized care plans
  • Collaborating with healthcare providers to ensure seamless care transitions
  • Providing education and support to patients and families on proper care techniques
  • Monitoring patient progress and making adjustments to care plans as needed

Required Skills and Qualifications

To be successful in this role, you will need:

  • A current unrestricted license to practice as a Registered Nurse in the state associated with this position
  • Current CPR/AED/BLS/First Aid certification
  • Reliable transportation to/from care sites and/or work locations
  • At least one year of professional experience practicing as a Registered Nurse in a home health or similar setting
