Care Manager PCC Job Description

Prime Care Coordination is a human services agency that partners with individuals with intellectual and/or developmental disabilities to provide personalized support and services.

Job Summary:

The Care Manager PCC is responsible for working in partnership with individuals to coordinate care and services needed to achieve optimal health, wellness, and life goals.

Key Responsibilities:
  • Conduct comprehensive assessments to identify individual needs, choices, and preferences for services
  • Assess and address health and safety issues, as well as barriers to care and treatment
  • Collaborate with interdisciplinary teams to develop and maintain person-centered Life Plans
  • Monitor and facilitate implementation of Life Plans, incorporating health promotion and support opportunities
  • Coordinate access to chronic disease management, referrals to clinical and community resources, and long-term care supports and services
  • Engage families and natural supports in the care coordination process, ensuring culturally and linguistically appropriate services
  • Advocate on behalf of individuals, promoting self-advocacy and self-direction
  • Use Health Information Technology for documentation, to link services, and facilitate communication among care coordination teams
Requirements:
  • Bachelor's degree with two years of relevant experience, or a license as a Registered Nurse with two years of relevant experience, or a Master's degree with one year of relevant experience
  • Ability to act quickly in crisis situations, basic technology skills, and knowledge of ethical and professional responsibilities and boundaries
