Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team of dedicated healthcare professionals. As a LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with our patients, families, and healthcare teams to ensure the best possible outcomes.

Responsibilities
  • Implement physicians' prescribed plan of care
  • Conduct patient assessments and coordinate care
  • Administer prescribed medication, treatments, and therapies
  • Document care provided in our system of record
  • Health promotion, teaching, and training of family members
  • Accept and write verbal orders from physicians as required
Requirements

• 6 months prior hands-on nursing experience preferred but not required
