The Step-Down Navigator plays a critical role in ensuring a seamless transition for patients from inpatient to outpatient services. This position is responsible for meeting with eligible patients to discuss the benefits of outpatient services and communicating eligibility to key stakeholders. The ideal candidate will have a strong understanding of the step-down process and be able to effectively communicate with patients, families, and healthcare teams.
Key Responsibilities- Meet with eligible patients to discuss outpatient services and eligibility
- Communicate eligibility to key stakeholders, including senior leadership and associated departments
- Routinely collect and enter data into Midas for trending report purposes
- Complete documentation in MS4 for inquiry calls related to outpatient services
- Assist with outcome reporting as needed
- Bachelor's degree from an accredited university required; Master's degree preferred
- Minimum of two years' experience in a psychiatric/mental health field, preferred
- Must have or be able to obtain a Level One Fingerprint Clearance Card
