We are seeking a highly skilled Assessment and Enrollment Nurse to join our team in New York State. As an Assessment and Enrollment Nurse, you will play a critical role in ensuring that our patients receive the highest quality care and support.

Salary: $85,000 - $105,000 + $15,000 Sign on Bonus, with opportunities for advancement and professional growth.

The Assessment and Enrollment Nurse will be responsible for conducting comprehensive assessments of patients' clinical, psychological, and social needs using the Uniform Assessment System (UAS-NY). They will determine eligibility for enrollment in our programs and develop individualized plans of care.

  • Conduct regular assessments of patients' needs and provide ongoing support.
  • Collaborate with healthcare professionals to ensure seamless coordination of care.
  • Develop and implement individualized plans of care to meet patients' unique needs.

Qualifications: To be successful in this role, you will need to have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment. You must also be certified in UAS-NY and have a minimum of one year's experience in community health or managed care.
