**Job Overview**

Bluegrass Care Navigators is seeking a skilled Registered Nurse to join our interdisciplinary team. As a RN, you will provide high-quality patient care and support to individuals with terminal illnesses, focusing on comfort and quality of life.

About the Role

The successful candidate will have a strong nursing background, excellent communication skills, and the ability to work autonomously in a dynamic environment. You will be responsible for assessing patient needs, developing individualized care plans, and collaborating with other healthcare professionals to ensure comprehensive support.

Key Responsibilities

  • Provide skilled nursing care to patients with terminal illnesses, prioritizing their physical and emotional comfort
  • Conduct thorough assessments of patient needs, identifying areas for improvement and developing targeted care plans
  • Collaborate with multidisciplinary teams to coordinate care, including physicians, therapists, and social workers
  • Document patient progress, ensuring accurate and timely records
  • Participate in ongoing education and training to enhance clinical knowledge and skills

Requirements

  • Current licensure as an RN in the state of practice
  • At least one year of general nursing experience, preferably in hospice, home health, or critical care settings
  • Demonstrated expertise in pain management, IV administration, and wound care
  • Excellent observation, communication, and documentation skills
  • Able to work independently, prioritize tasks, and manage time effectively
