About the Role

This position is responsible for delivering exceptional patient care, promoting health and well-being, and collaborating with interdisciplinary teams to achieve optimal outcomes. As a Patient Care Specialist, you will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans that address physical, emotional, and social needs.

Key Responsibilities
  • Assess patient needs and develop individualized care plans
  • Deliver high-quality patient care, ensuring safety, comfort, and dignity
  • Educate patients and families on healthcare management, disease prevention, and wellness strategies
  • Collaborate with interdisciplinary teams to coordinate care and promote continuity
  • Document patient information accurately and efficiently, using electronic medical records systems
Requirements

To be successful in this role, you will need:

  • A degree in Nursing or a related field
  • Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN)
  • Experience working in a clinical setting, preferably in a hospital or healthcare facility
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently
