About the Role

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team in Denver, CO. As a member of our Home-based Medical Care team, you will play a vital role in providing high-quality medical care to complex, chronic patients in their own homes.

Responsibilities

  • Provides medical care and management to members served by the program, working with the Landmark Health Medical Director, Director of Health Services, and UM staff to develop and implement medical management policies and clinical protocols.

  • Establishes and maintains effective relationships with members, facility staff, PCPs, and other collaborating physicians/providers, performing initial comprehensive assessments and developing member management care plans.

  • Counsels members and/or their legal representatives on disease processes affecting members and ways to manage them effectively, promoting wellness and educating members on cost-effective alternatives regarding patient care delivery systems.

  • Supports FCMs in developing and providing in-service educational programs to facility staff, participates in management meetings, and implements HEDIS measure campaigns and quality initiatives as directed by Corporate Health Services.

Requirements

  • Completion of Nurse Practitioner accredited program.

  • Current state RN license and NP license.

  • Knowledge of long-term care, community resources, and CPT, ICD-9, and HCPC codes.

  • Computer skills, including proficiency in Windows 2000 and MS Office Word Processing software.

  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.

  • Ability to write reports, business correspondence, and procedure manuals.

  • Access to reliable transportation.

Benefits

As a valued member of our team, you will enjoy a competitive salary range of $88,000 to $173,200 annually, plus a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (all benefits are subject to eligibility requirements).
