Job Summary

We are seeking a highly skilled Senior Physical Rehabilitation Specialist to join our team at Phoenix Home Care and Hospice. This is a full-time position with a competitive salary range of $85,000 - $100,000.

About the Role

The successful candidate will provide physical therapy services to patients in their homes, working closely with physicians and other healthcare professionals to ensure optimal care. Our ideal candidate is a motivated and compassionate individual with excellent communication skills and a passion for delivering high-quality patient care.

Responsibilities
  • Provide physical therapy services to patients in their homes, following physician orders and agency protocols.
  • Participate in multidisciplinary team meetings to discuss patient progress and develop comprehensive care plans.
  • Conduct thorough assessments and create tailored treatment plans to address each patient's unique needs.
  • Maintain accurate records and communicate effectively with patients, families, and healthcare teams.
  • Collaborate with colleagues to ensure seamless care transitions and optimal patient outcomes.
Requirements
  • Degree in Physical Therapy from an accredited institution.
  • Active license to practice as a Physical Therapist in the state of MO.
  • Home health experience preferred but not required.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
