Job SummaryWe are seeking a skilled and compassionate Hospice Registered Nurse (RN) to join our team as a Case Manager. In this role, you will be responsible for providing high-quality patient care, collaborating with interdisciplinary teams, and advocating for the needs of our patients and their families.
Key Responsibilities- Provide direct patient care services, including assessments, care planning, and implementation of individualized plans of care.
- Collaborate with physicians, nurses, social workers, and other healthcare professionals to ensure comprehensive care coordination.
- Empower patients and their families by educating them on care options, resources, and support services.
- Manage patient records, ensuring accurate and timely documentation of care provided.
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices.
- RN license in good standing, with a minimum of 1 year of experience in hospice, home health, or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently, with a high degree of autonomy and flexibility.
- Current CPR certification and a valid driver's license.
