As a RN Case Manager, you will be responsible for providing high-quality patient care in a home setting. Your duties will include skilled nursing care, patient assessments, and developing plans of treatment.

Responsibilities:
  • Provide skilled nursing care, including wound care, catheter care, and medication compliance
  • Assess and supervise patient care, ensuring chart compliance with state regulations

Requirements:
