About the Role

We are seeking a compassionate and skilled Home Health Registered Nurse to join our extraordinary care team at Residential Home Health and Hospice. As a Home Health RN, you will lead and collaborate with our team to provide management and delivery of patient care plans, grounded in our belief that outstanding care is best delivered in a team-based environment.

Key Responsibilities
  • Lead and collaborate with the care team to develop and implement patient care plans
  • Provide high-quality patient care and education to patients and their families
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care plans
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery

Requirements
  • RN license in the state you work
  • Graduate from an approved school of practical nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones and EMR software
