As a Home Health Registered Nurse, you'll play a vital role in our team-based environment, working closely with our experienced care team to develop and implement personalized patient care plans.

We're seeking compassionate and skilled RNs to join our team, with a focus on delivering high-quality patient care and achieving exceptional outcomes. Our comprehensive onboarding program and ongoing support ensure that you'll have the tools and resources you need to succeed in this rewarding role.

Key Responsibilities:
  • Lead and collaborate with our care team to develop and implement patient care plans
  • Provide high-quality patient care, including assessments, interventions, and education
  • Communicate effectively with patients, families, and healthcare providers
  • Participate in ongoing education and professional development to stay up-to-date on best practices

Requirements:
  • RN license in the state you work
  • Graduate from an approved school of practical nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
