You will work under the guidance of physicians and be responsible for conducting screenings, evaluations, and treatment plans for residents.

Responsibilities:

* Provide Speech-Language Pathologist services to residents and initiate referrals as needed
* Conduct screenings and evaluations to determine the need for intervention/treatment
* Develop and implement treatment plans to attain goals in accordance with physician orders
* Monitor resident's response to intervention and modify treatment as indicated
* Develop home or community programming to maintain and enhance resident performance
* Provide clinical information in a timely manner concerning safety and functional needs for discharge
* Adhere to established confidentiality standards
* Implement and conduct treatment as outlined in treatment plan
* Follow management direction
* Participate in Continuous Quality Improvement program as directed

Requirements:

* Master's degree in Speech-Language Pathology from an accredited program
* Successful completion of National Certification Examination for S.L.P.
* Current state licensure or license eligible
* 0-3 years of long-term care experience
