As an LPN with Aveanna, you will have the opportunity to work in a dynamic and supportive environment, delivering care that makes a meaningful difference in the lives of our patients and their families.
Key Responsibilities- Provide skilled nursing care to patients with tracheostomy and ventilator needs, under the supervision of a Registered Nurse.
- Develop and implement individualized care plans, in collaboration with the patient's healthcare team.
- Conduct thorough assessments of patient needs, identifying areas for improvement and implementing interventions to achieve optimal outcomes.
- Document all patient interactions, assessments, and care provided, in accordance with Aveanna's policies and procedures.
- Collaborate with the patient's healthcare team to ensure seamless transitions of care and optimal patient outcomes.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Must be able to speak, write, read, and understand English.
- Must be able to travel.
- Must be able to lift 50 pounds.
- Must be able to sufficiently reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Must be able to sit and climb stairs.
- Must have visual and hearing acuity.
- Must have a strong sense of smell and touch.
