Job Summary:The Licensed Practical Nurse (LPN) will be responsible for providing and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders for each individual patient. This includes:
- Delivering and coordinating quality patient care in compliance with physician orders.
- Observing and assessing patient condition and care needs, and reporting changes to the supervisor and/or physician as appropriate.
- Documenting all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participating in and updating the nursing care plan.
- Taking appropriate nursing action based on assessment and achieving expected outcomes.
To be successful in this role, you will need:
- A graduate degree from an accredited school of nursing.
- A current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
While not required, we prefer candidates with:
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
This role requires:
- The ability to speak, write, read, and understand English.
- The ability to travel.
- The ability to lift 50 pounds.
- The ability to sufficiently reposition patients and move equipment without assistance.
This role involves working in a variety of environments, which may include exposure to allergens and other conditions. You may also be exposed to blood, bodily fluids, and infectious diseases.
