Position Summary

Shift: 8:00 AM – 4:30 PM

No weekends, holidays, or on-call duties

  • National recognition in ventricular assist devices and biventricular pacemaker therapy
  • First in the nation to achieve Patient-Centered Specialty Practice Level 3 Recognition from the National Committee for Quality Assurance, the highest level for specialty practices.

We are looking for a dedicated Pre-Transplant Coordinator (Level II), an experienced cardiovascular nurse who provides comprehensive patient care throughout the transplant journey.

  • Collaborates with a multidisciplinary team to streamline patient flow from initial referral through evaluation, education, and post-transplant follow-up.
  • Works closely with physicians, surgeons, and case managers to facilitate the transplantation process.
  • Identifies, evaluates, and addresses clinical issues related to transplant patients following established protocols.
  • Engages with staff, patients, and families regarding evaluation, discharge planning, and long-term care.
  • Maintains accurate documentation for transplant candidacy and post-transplant follow-up in compliance with regulatory requirements.
  • Delivers necessary education throughout the transplant process to candidates, families, and significant others.
  • Ensures completion of all evaluation requirements for presentation at weekly Selection Conferences.
  • Prepares and communicates patient evaluation findings effectively to a multidisciplinary team during Selection Conferences.
  • Possesses excellent written and verbal communication skills.

Minimum Qualifications:

  • Associate's degree in nursing.
  • A minimum of 4 years of recent clinical experience in medical-surgical nursing, including at least one year in a transplant coordinator role.
  • Active nursing license or temporary permit approved by the relevant licensing board. BLS certification is required.

Preferred Qualifications:

  • Bachelor's Degree in Nursing.
  • Experience in Critical Care or Cardiology.
