Job Summary

The Behavioral Health Professional provides high-quality assessments of health home clients' care needs and facilitates timely access to care. This position oversees care plans of clients in the panel and lends clinical judgment to team members as needed.

Key Responsibilities
  • Conducts initial comprehensive assessments of new and returning clients to the health home, using agency-identified assessment tools and baseline measures.
  • Completes initial care plans and provides oversight of care plan updates.
  • Maintains clinical oversight of clients assigned to the Behavioral Health Home and signs off on documentation from other team members as needed.
  • Triages crisis situations and identifies high-risk or urgent clients at assessment and provides triage to get clients in quickly for services.
  • Assesses any needs for special reports, letters, or applications and delegates tasks to Client Service Advisors as needed.
  • Reviews care plans and treatment progress with Care Coordinators and identifies any next steps or follow-up needed.
Requirements
  • Must have a strong understanding of behavioral health principles and practices.
  • Must be able to work effectively in a team environment and communicate effectively with clients and team members.
  • Must have strong clinical judgment and problem-solving skills.
