We are seeking a skilled Clinical Care Coordinator to join our team at A Special Touch Home Health Care LLC. The successful candidate will be responsible for the assessment, diagnosis, and treatment of assigned patients.

Responsibilities:
  • Provide high-quality nursing care to ill, injured, or disabled patients
  • Develop comprehensive patient treatment plans in collaboration with healthcare professionals
  • Monitor and report changes in patient symptoms or behavior to ensure optimal care
  • Foster effective communication with collaborating physicians and specialists to coordinate patient care
  • Educate patients on health maintenance and disease prevention strategies
  • Facilitate referrals to other healthcare professionals and medical facilities as needed
  • Maintain accurate and up-to-date patient medical records
  • Offer advice and emotional support to patients and their family members during challenging times

The estimated salary for this position is $85,000 - $110,000 per year, depending on experience and qualifications.
