Nurse Care Coordinator

We are seeking a highly skilled and compassionate Registered Nurse to join our team at the Chesapeake Tricare Prime Clinic. This is a full-time or PRN opportunity to provide exceptional patient care and contribute to the success of our healthcare organization.
About the Role- Coordinate patient care in collaboration with healthcare professionals to achieve optimal outcomes in clinical care, quality, and cost-effectiveness.
- Ensure compliance with standards of care and practice according to established policies, procedures, and guidelines.
- Perform physical exams and health histories to identify patients' needs and develop personalized care plans.
- Provide health promotions, counseling, and education to empower patients to take control of their health.
- Administer medications, wound care, and other interventions as needed to support patients' recovery and well-being.
- Direct and supervise the care provided by other healthcare professionals to ensure high-quality patient care.
- Degree: Associate degree in Nursing from an accredited institution.
- Experience: One year of experience in nursing, preferably in a similar role or setting.
- Licensure: Current, active, and unrestricted license to practice as a Registered Nurse.
