Key Responsibilities:
Monitor chronic care logs and manage caseloads to facilitate staff referrals.

Establish discharge planning referral criteria for intake medical screeners.

Regularly consult with medical providers regarding current caseloads to identify patients eligible for pre-release health planning.

Collaborate with Armor's behavioral health professionals, including psychiatrists and licensed mental health experts, for patients with complex needs requiring additional coordination beyond individual treatment plans or behavioral health referrals upon release.

Engage in multidisciplinary planning to support specialized transitional programs for seriously or chronically ill patients, such as community-based mental health teams and HIV Jail Linkage initiatives.

Qualifications:
A minimum of two years of experience in correctional health or criminal justice settings as a case manager, discharge planner, nurse, or mental health service coordinator.

Bachelor's Degree in social sciences, human services, healthcare, or a Nursing Degree.

Strong written and verbal communication skills; proficient computer skills.

Familiarity with health records, medical terminology, and health information services.
