Home Health LPN Position

We are seeking a skilled Licensed Practical Nurse (LPN) to provide high-quality in-home care services to our patients in Snohomish county, WA.
Responsibilities- Provide skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseload.
- Review caseload with supervising RN on a regular basis as established by best practice and/or State guidelines.
- Obtain necessary physician orders and collaborate with the physician as the patient's needs dictate.
- Evaluate the significance of assessment findings and communicate pertinent information about the patient's clinical status and ongoing needs to others in a timely fashion.
- Guarantee that changes in patients' clinical status are communicated to other healthcare team members, necessary office staff, and physician as per Agency policy.
- Educate patients, caregivers, families, and other staff as appropriate.
- Perform aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Participate in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
- Recommend for needed referrals to other disciplines and/or community resources.
- Follow the seven medication rights and reduce the potential for medication errors.
- Follow up with obtaining lab results and notify the physician of significant changes.
- Participate in the agency Quality Assurance Performance Improvement (QAPI) program.
- Identify risk of injuries or threats to life and health in the patient's environment (e.g. environmental/safety/mobility risks) and implement appropriate interventions to reduce those risks.
- Complete accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashion.
- Graduate of an accredited nursing program.
- Current licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practice.
- Experience as an LPN/LVN commensurate with one of the following preferred:
- One year experience in an acute care setting within the last 24 months;
- Six months home care experience within the last 12 months preferred.
- Valid driver's license and reliable transportation
- Proof of automobile insurance (MVR checked)
- Proof of current CPR certification
- Able to pass criminal background check and national sex offender clearance
