Job Summary

We are seeking a highly skilled Pharmacy Technician to join our Transitions of Care Pharmacy team. As a Medication Historian Technician, you will play a critical role in gathering information about patients' medications and compiling a complete list of their home medications. This is a full-time position with variable shifts.

Responsibilities
  • Practice at the top of your license and expand your skill set
  • Combine clinical pharmacy practice with advanced patient care processes
  • Actively participate in staff meetings and work independently with great accuracy and attention to detail
  • Identify patients being admitted through the emergency department and gather information about their medications
  • Input all medication history information into the electronic health record
  • Keep record of all work processed for intervention tracking and reporting
Qualifications
  • High school diploma or equivalent (GED)
  • Valid Idaho Pharmacy Technician License
  • Preferred Qualifications: Two (2) + years' pharmacy technician experience and Certified Pharmacy Technician (CPhT)
