RN Long Term Care Specialist

The Registered Nurse (RN) is responsible for delivering high-quality patient care to residents in a long-term care setting. This role involves utilizing the nursing process to assess, diagnose, plan, implement, and evaluate care for residents. The RN will work collaboratively with residents, families, and other healthcare professionals to ensure comprehensive care.
Responsibilities- Provide individualized nursing care to residents, including assessment, medication administration, and wound care.
- Develop and implement individualized care plans in collaboration with residents, families, and other healthcare professionals.
- Collaborate with the interdisciplinary team to ensure seamless care transitions and coordination.
- Monitor and report changes in resident condition to the healthcare team.
- Participate in quality improvement initiatives to enhance resident care and satisfaction.
- Graduate from a nationally accredited nursing program.
- Currently holds an unencumbered RN license in the state where the practice of nursing is occurring.
- Obtains and maintains required department-specific competencies and certifications.
