Job Summary

The Licensed Practical Nurse (LPN) provides high-quality nursing care for residents of all ages in a long-term care setting, under the supervision of a registered nurse, advanced practice provider, or physician.

Key Responsibilities:

  • Delivers nursing care that is patient-centered, compassionate, and respectful of individual needs and preferences.
  • Conducts thorough assessments of residents' physical and psychosocial needs, develops and implements individualized care plans, and evaluates their effectiveness.
  • Administers medications and treatments as ordered, monitors residents' responses, and reports any changes or concerns to the supervising nurse or physician.
  • Provides technical support to healthcare professionals, documents resident interactions and outcomes, and maintains accurate and confidential records.
  • Counsels residents, family members, and/or resident advocates on prevention and treatment plans, and educates them on self-care programs.

Requirements:

  • Graduate from an accredited practical nursing program as a Licensed Practical Nurse.
  • Currently holds an unencumbered Licensed Practical Nurse (LPN) license with the State Nursing board and/or possesses multi-state licensure privileges.
  • Obtains and maintains required department-specific competencies and certifications.
