- Conduct initial and ongoing patient assessment, analyze assessment data, and create a plan of care.
- Administer medications safely and consistently with state and Bon Secours policies.
- Serve as a point of contact for patient care coordination throughout hospital departments.
- Act as a patient safety advocate and participate in quality improvement initiatives.
- Process orders, review medication appropriateness, and perform clinical support duties as needed.
Requirements:
- Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD.
- Advanced pharmacy degree preferred.
- Current license for pharmacist in the state(s) where practicing.
- Valid state-issued ID.
- BLS Basic Life Support preferred.
- Immunization Certified preferred.
Preferred Qualifications:
- 2 years of recent experience in an acute care setting.
- 1 year of leadership experience.
- Experience in the field of pharmacy practice.
