Hospice Care Nurse

Liberty Health is seeking a skilled Hospice Care Nurse to join our team.
We understand the unique needs of our patients and families facing terminal illness. Our goal is to provide state-of-the-art care and pain management services, delivered by specially trained staff with an emphasis on strength, dignity, and compassion.
As a Hospice Care Nurse, you will be responsible for:
- Applying your knowledge of nursing theories and concepts.
- Caring for and documenting patient care that meets reimbursement guidelines.
- Coordinating physical care with the patient, family, physician, and hospice team.
- Communicating effectively with patients, families, and agency personnel.
This role requires a current RN licensure in the state of practice, successful completion of a nursing education from an approved school of nursing, two years of clinical nursing experience, and at least one year of hospice nursing experience preferred. You should have a working knowledge of the principles and practice of nursing according to the nursing practice act, agency policies, and procedures, as well as reimbursement guidelines.
The estimated annual salary for this position is around $85,000-$110,000, depending on location and experience.
