Hospital Care Specialist

We are seeking a skilled Hospital Care Specialist to join our hospice team in Monroe, North Carolina. As a Registered Nurse, you will be responsible for delivering high-quality patient care and coordinating with multidisciplinary teams to ensure seamless transitions between healthcare settings.
About the Role- You will be working closely with patients, families, and caregivers to understand their unique needs and preferences, providing personalized support and guidance throughout their journey.
- As a Hospital Care Specialist, you will assess patients' conditions, develop individualized care plans, and implement interventions to improve health outcomes and quality of life.
- You will also work collaboratively with our interdisciplinary team to identify areas for improvement and implement evidence-based practices to enhance patient care.
- A graduate degree from an accredited nursing program is required; BSN preferred.
- Current RN license or temporary license as a Registered Nurse Petitioner in the state of North Carolina is mandatory.
- BLS certification per policy guidelines is also required.
- Competitive salary: $83,000 - $110,000 per year, depending on experience.
