Position: Residential Support Coordinator

General Summary: The Residential Support Coordinator is responsible for delivering direct services, coordinating care, and facilitating access to resources and advocacy for clients and their families. The primary goal is to assist clients in managing their lives and supporting their recovery journey.
Key Responsibilities:
- Conduct assessments, provide referrals, and offer education and follow-up services to clients regarding mental health, medical needs, substance use, vocational skills, and daily living capabilities.
- Collaborate with various service providers, agencies, and individuals within the client's support network.
- Educate the community on issues related to mental health and substance use disorders.
- Monitor clients' psychiatric health, oversee medication adherence, and document observations, reporting any discrepancies to the appropriate supervisory personnel.
- Teach clients effective strategies for managing symptoms and adhering to prescribed medication.
- Assist clients in building and enhancing their support systems to achieve personal goals.
- Support clients in securing safe and affordable housing and managing their financial resources.
- Guide clients in the administration of their representative payee accounts.
- Develop and implement individualized recovery plans, ensuring documentation aligns with organizational standards.
- Complete and maintain necessary billing forms, logs, and clinical documentation in compliance with organizational and regulatory requirements.
- Schedule client appointments to meet individual and program billing objectives.
- Participate in clinical team meetings to ensure effective coordination of client services.
- Provide crisis intervention as needed.
- Accompany clients to community appointments, offering advocacy as required.
- Assist in the coordination of clients' physical health care needs.
- Regularly check for new communications to stay updated on client needs.
Additional Duties:
- Attend all required training sessions and departmental meetings.
- Participate in safety training programs and understand responsibilities related to workplace safety.
- Demonstrate respect and professionalism towards clients, families, visitors, and colleagues.
- Uphold the organization's commitment to diversity, equity, and inclusion.
- Maintain a professional appearance in accordance with organizational policies.
- Adhere to assigned work hours and punctuality.
- Foster a positive organizational image in line with the company's guiding principles.

Qualifications:
- Bachelor's Degree in a Human Services field or equivalent combination of education and experience.
- At least two years of experience in the Human Services sector, particularly with clients with disabilities.
- Familiarity with state and federal assistance programs is preferred.
- Basic computer skills are essential.
Certifications and Licenses:
- Valid driver's license and automobile insurance meeting state minimum requirements, or the ability to secure necessary transportation.
- Case Management/Community Support Professional Certification or progress towards certification is preferred.
Physical Requirements:
The physical demands of this role include standing, walking, sitting, using hands for various tasks, reaching, climbing stairs, and occasionally lifting up to 25 pounds. Reasonable accommodations may be made for individuals with disabilities to perform essential functions.
