**Job Summary**

We are seeking a dedicated Home Health Care Professional to provide exceptional care and support to our patients at home. Your role will involve assisting with daily activities and ensuring the well-being of our patients.

**Key Responsibilities**

  • **Personal Care**: Assist with hygiene, bathing, and dressing.
  • **Mobility**: Help with physical activities and exercises.
  • **Meals**: Aid in meal planning and preparation.
  • **Housekeeping**: Perform light housekeeping tasks.
  • **Reminders**: Assist with medication and appointments.
  • **Errands**: Accompany patients with shopping.

**Requirements**
