Position Overview

Great Lakes Integrated Network, Inc. is in search of a Social Worker - Oncology to provide essential support to oncology patients, their families, and caregivers. This role focuses on delivering comprehensive information regarding available resources, addressing financial, practical, and emotional challenges throughout the care continuum. The successful candidate will collaborate with the oncology multidisciplinary team to eliminate healthcare barriers and ensure that services are aligned with professional standards.

Key Responsibilities

The responsibilities of this position include, but are not limited to:

  • Facilitating psychosocial assessments for patients and their families during critical medical appointments, covering all stages from prevention to end-of-life care.
  • Offering counseling and educational resources related to cancer diagnosis and treatment, while coordinating referrals to relevant disciplines and community support.
  • Identifying and addressing barriers to care, ensuring coordination with appropriate resources.
  • Engaging in the Cancer Focused Community Needs Assessment working group.
  • Collaborating with the Manager of Survivorship & Supportive Services to develop programs aimed at enhancing patients' quality of life and outcomes throughout their treatment journey.
  • Evaluating service gaps within the cancer program and local agencies.
  • Creating assessment tools that consider psychosocial, social, cultural, and spiritual factors.
  • Participating in multidisciplinary center visits to provide education and support regarding psychosocial matters.
  • Acting as the Psychosocial Services Coordinator during Commission on Cancer meetings.
  • Facilitating referrals to the Survivorship provider and program as necessary.
  • Collaborating with team members on the Patient Advisory Committee.
  • Assessing and providing guidance on various topics, including:
    • Substance abuse and smoking cessation programs
    • Lodging assistance during treatment
    • Programs for caregivers and family members
    • Social Security Disability
    • Medicaid and Medicare guidelines
    • Financial assistance programs
    • Transportation resources
    • Advance directives and healthcare proxies
    • Palliative and hospice care resources
    • Medication assistance programs
Qualifications

To be considered for this role, candidates should possess:

  • Experience: A minimum of two years in medical social work is required, with oncology experience preferred.
  • Licenses and Certifications:
    • Master's degree in Social Work
    • Current LCSW or LMSW licensure
    • Oncology Social Worker Certification (OSW-C) to be obtained within three years of hire
    • Valid driver's license for travel between multiple sites
Skills and Competencies

The ideal candidate will demonstrate:

  • Proficiency in timely and accurate documentation using designated technology platforms.
  • Adaptability to new technologies and software.
  • Basic computer skills (MS Word, Outlook, PowerPoint, Excel).
  • Knowledge of federal and state regulations regarding Medicare and Medicaid.
  • Strong interpersonal skills, including empathy, active listening, and crisis management.
  • Effective problem-solving abilities and teamwork orientation.
  • Excellent written and verbal communication skills.

This position requires onsite work at various locations, with some travel involved.

Compensation and Benefits

We offer a competitive benefits package, including health and dental insurance, a 401K plan, vacation, and paid time off. The salary range for this position is between $65,000 and $80,000, determined by factors such as market data, education, experience, and qualifications.
