Key Responsibilities:

  • Provide high-quality patient care, including assessment, diagnosis, treatment, and management of acute and chronic health needs.
  • Lead a care team consisting of care promoters, care facilitators, and care coordinators to ensure seamless patient care.
  • Engage with hospitalists, case managers, and other healthcare professionals to ensure comprehensive patient care.
  • Participate in quality improvement initiatives and contribute to the development of best practices.
  • Collaborate with center and market leadership to achieve departmental goals and objectives.

Requirements:

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
  • Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty.
  • Current DEA number for schedule II-V controlled substances.
  • Basic Life Support (BLS) certification from the American Heart Association (AHA) or American Red Cross (ARC).
