Job Title: Licensed Practical Nurse (LPN)

Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Interim HealthCare of the Twin Cities. As an LPN, you will provide high-quality patient care in a home setting, working closely with our interdisciplinary team to deliver exceptional healthcare services.

Responsibilities:

  • Participate in ongoing interdisciplinary assessment of the patient.
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
  • Provide services that are ordered by the physician as indicated in the plan of care.
  • Recognize and report changes in the patient's condition to the RN.
  • Counsel and educate the patient and family in meeting healthcare-related needs.
  • Complete timely and accurate clinical notes, including addressing patient progress.

Requirements:

  • Current license as an LPN in the State of MN.
  • 2 years' experience as an LPN.
  • Able to travel within geographic area serviced by the office.
  • CPR Certification.
