**Job Summary**

We are seeking a skilled Clinical - LTSS Service Care Manager GA to join our team at Axelon Services Corporation. As a remote employee, you will be working with the Star Plus Waiver member population.

The ideal candidate will have strong experience in reviewing assessments and service plans, as well as researching community resources. They will also be responsible for terminating LTSS waiver service authorizations, providing community resources, and initiating LTSS/acute services.

This role requires excellent communication skills, attention to detail, and the ability to work independently. The salary range for this position is $55,000 - $70,000 per year, depending on experience.

Key Responsibilities:

  • Review assessments and service plans
  • Research community resources
  • Terminate LTSS waiver service authorizations
  • Provide community resources
  • Initiate LTSS/acute services
  • Research complaints and provide updates to managers
  • Verify all services have been started
