Job Description

Sea Mar Community Health Centers is seeking a skilled Case Manager to join its Behavioral Health department in Seattle, WA. The ideal candidate will provide primary community support and case management to youth and families struggling with mental health issues.

Key Responsibilities:

  • Conduct intake screenings to determine client needs and develop a Client Service Plan.
  • Make referrals to appropriate agencies or services and provide advocacy as needed.
  • Maintain regular contact with clients and provide support to encourage cooperation in implementing the Service Plan.
  • Assist clients in accessing services, such as filling out forms and obtaining necessary documentation.
  • Participate in interdisciplinary team case reviews and promote a positive team approach.

Requirements:

  • Associate's degree in Behavioral Health, Health Sciences, or related field.
  • Two years of closely related work experience or equivalent.
  • Eligibility for Agency Affiliated Counselor Registration from the State Department of Health.
  • Fluency in Spanish and English, with the ability to write routine reports and correspondence.
