Responsibilities:
  • Assess medication therapy regimens and develop personalized plans to optimize patient outcomes.
  • Collaborate with healthcare professionals to ensure seamless medication management and minimize errors.
  • Participate in quality improvement initiatives and contribute to research activities to advance the field of pharmacy.
  • Provide education and support to patients and families on medication use and adherence.
Qualifications:
  • Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.
  • Current pharmacist license by the Board of Pharmacy in the state(s) of the job's practice site, or eligible to obtain within a time frame compliant with the site's applicable employee licensure policy.
