Job Summary

UnitedHealth Group is seeking a skilled RN Case Manager to join our team in a field-based role. As a RN Case Manager, you will play a critical role in assessing, planning, and implementing care strategies for Medicare and Medicaid members. Your expertise will be utilized to proactively engage members in managing their own health and healthcare, ensuring they receive the most appropriate level of care.

Key Responsibilities
  • Assess and develop individualized care plans for members with complex medical and behavioral needs
  • Utilize strong communication and interpersonal skills to engage members in their care and provide education on health-related topics
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and identify areas for improvement
  • Manage transitions of care for members upon discharge from in-patient hospital stays and emergency rooms
  • Provide clinical support to non-clinical staff, including education on medications, diagnoses, and disease-specific topics
Requirements
  • Current, unrestricted RN license in the state of Kansas
  • 2+ years of experience working in the community as a RN in acute inpatient care, long-term care, home health, hospice, public health, behavioral health, or assisted living
  • Intermediate level of proficiency working with MS Word, Excel, and Microsoft Outlook
  • Ability to travel up to 75% of the time with reliable transportation, a current and non-restricted state of Kansas Driver's License, and State-required insurance
Preferred Qualifications
  • Bachelor's degree in nursing
  • Background in managing Medicaid and Medicare populations with complex medical and behavioral needs
  • Experience with arranging community resources
  • Experience with electronic charting
