Key Responsibilities:- Provide program orientation to patients, families, and caregivers, including an overview of the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting their strengths, abilities, needs, and preferences.
- Facilitate the involvement of the patient throughout the rehabilitation process.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, and caregiver.
- Registered Nurse Diploma
- Experience in rehabilitation care and case management
- Strong communication and interpersonal skills
- Ability to work collaboratively with patients, families, and healthcare teams
- Knowledge of rehabilitation principles and practices
