Position Summary:
The RN Case Manager plays a vital role in ensuring that patients transition from hospital to home or other care settings safely and effectively. This position involves identifying and addressing any potential obstacles to a smooth discharge process in collaboration with the healthcare team.
Key Responsibilities:
1. Conduct thorough assessments to determine patients' discharge planning needs in partnership with an interdisciplinary team.
2. Ensure adherence to discharge planning regulations and guidelines.
3. Identify patients at high risk for readmission and develop comprehensive care plans to mitigate these risks.
4. Collaborate with team members to enhance standard practices and improve patient outcomes.
5. Engage in utilization management and act as a liaison with post-acute care providers.
Qualifications:
Education: Graduate of an accredited nursing program.
Experience: Minimum of three years in an acute care setting.
Licensure: Current Registered Nurse license in the state of practice.
Physical Requirements:
Ability to perform frequent lifting, carrying, and moving of patients and equipment. Requires good vision, hearing, and communication skills to effectively interact with patients and healthcare team members.
