Overview
The Licensed Practical Nurse (LPN) will execute responsibilities under the guidance or oversight of a licensed healthcare provider or a registered nurse.
The LPN is tasked with performing a focused evaluation of the patient, which includes monitoring vital signs and collecting relevant information, implementing established care plans, and assessing the effectiveness of treatments within the healthcare environment.
Key Responsibilities
- Consistently complete all necessary clinical documentation in accordance with Riverside Health System standards. Maintain documentation for quality assurance, including but not limited to point of care testing, medication logs, and annual wellness visits. Collaborate with providers to ensure care gaps are routinely addressed for each patient. Ensure documentation is accurate, legible, and completed on the same day. Demonstrate proficiency in laboratory functions. Monitor OSHA compliance and lab proficiency testing. Maintain clean, compliant, and well-stocked work areas.
- Provide patient education as required. Instruct patients on the medications prescribed to them. Demonstrate effective patient care by clarifying the importance of completing registration and medical history forms and how this relates to their care at our facility. Follow up with patients urgently regarding lab results, phone calls, prescription fills and refills, referrals, and appointment scheduling.
- Respond to telephone inquiries from patients, pharmacies, physicians, vendors, etc., by the end of the business day. Communicate lab/pathology results and schedule additional tests as per provider orders.
- Ensure proper reimbursement from insurance companies by performing pre-authorization for medications, procedures, and patient tests.
- Exhibit the ability to foster trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively engage in multi-disciplinary team meetings as assigned.
Education
- Graduate of a Licensed Practical Nurse program (Preferred)
- Bachelor's Degree in Nursing (Preferred)
- Minimum of 1 year of nursing experience (Preferred)
- Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
- CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire (Required)
