Commitment: Team members are expected to have a minimum scheduling commitment of 8 hours per week or 24 hours per week.

Role Overview:
As a Licensed Practical Nurse (LPN), you will operate under the guidance of the office manager, RN, clinical lead, and physicians. Your primary responsibility will be to assess, plan, implement, and evaluate comprehensive nursing care for assigned patients. The LPN will engage in various roles within Riverside Medical Group, delivering hands-on clinical care, including vital signs assessment, medication reviews, injections, and blood draws. Utilizing effective communication, critical thinking, and problem-solving skills, you will develop individualized care plans that consider the physiological, psychosocial, and cultural diversity of patients. Additionally, you may supervise and delegate tasks to non-professional staff to ensure optimal patient care. It is essential to demonstrate proficiency in addressing the age-related needs of specific patient populations within your assigned unit or department. You will collaborate with leadership and a multidisciplinary team to provide a wide range of clinical services aligned with the standards of excellence set by Riverside Health System. Flexibility to float to multiple locations within the division to provide support as needed is required.

Key Responsibilities:

  • Ensure accurate completion of all required clinical documentation in accordance with Riverside Health System standards. Maintain quality assurance documentation, including point-of-care testing, medication logs, and annual wellness visits. Collaborate with providers to address care gaps for each patient, ensuring documentation is precise, legible, and completed on the same day. Exhibit knowledge and skills in laboratory functions while monitoring OSHA and lab proficiency testing. Maintain clean, compliant, and well-stocked work areas.
  • Provide patient education as necessary, guiding patients on prescribed medications and emphasizing the importance of completing registration and medical history forms related to their care. Ensure timely follow-up with patients regarding lab results, phone calls, prescription fills, referrals, and appointment scheduling.
  • Build and maintain trusting collaborative relationships with a multidisciplinary team to support patient care needs. Actively participate in multidisciplinary team meetings as assigned.
  • Facilitate proper reimbursement from insurance companies by conducting pre-authorizations for prescription medications, procedures, and patient tests.
  • Respond to telephone inquiries from patients, pharmacies, physicians, and vendors by the end of the business day. Communicate lab and pathology results and schedule additional tests as directed by the provider.
Qualifications:

Education:
  • Graduate of a Licensed Practical Nurse program (Required)
  • Bachelor's Degree in Nursing (Preferred)
Experience:
  • 1 year of nursing experience (Preferred)
  • 1 year of outpatient medical office experience (Preferred)
  • 1 year of experience using electronic medical records (EMR) (Preferred)
  • Less than 1 year of experience in specimen collection and processing in a laboratory setting (Preferred)
Licenses and Certifications:
  • Licensed Practical Nurse (LPN) - Virginia Department of Health Professions (VDHP) Upon Hire (Required)
  • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire (Required)
