Job Summary:
Thom Child and Family Services is seeking a highly skilled and compassionate Registered Nurse to join our team as an Early Intervention Registered Nurse. As a key member of our multidisciplinary team, you will play a vital role in providing home and community-based early intervention services to infants and toddlers who have or are at risk for developmental delays and their families.
Key Responsibilities:
- Conduct initial intakes and provide follow-up referrals and/or interagency service coordination as needed.
- Participate in multidisciplinary team assessments to determine the developmental needs of the child and concerns and priorities of the family.
- Develop and implement Individual Family Service Plans (IFSPs) with families, collaborating with them to identify strengths and resources.
- Provide services using a coaching model with children and families according to their IFSPs, including home visits, child groups, and parent groups.
- Monitor developmental milestones and progress with parents/caregivers to inform ongoing need for direct service, consultation, and/or assessment.
- Provide parent/caregiver education on health and safety issues during home visits or parent education groups.
- Document all information necessary for the upkeep of client files.
- Participate in developmental screenings in the community.
- Attend program functions for clients and the public.
Requirements:
- Current Registered Nurse licensure in the state of Massachusetts.
- Ability to work independently and as a team member.
- Strong ability to engage in meaningful, therapeutic relationships with children, parents/caregivers, and community partners.
- Knowledge of family systems and early childhood development.
- Commitment to delivering individualized and culturally informed services.
- Excellent communication and organizational skills.
- Abide by all confidentiality and professional boundaries standards.
- Good computer skills as well as strong documentation skills.
- Current driver's license and reliable transportation.
- Bilingual/Bicultural a plus.
