Your Responsibilities:

  • Conduct admissions and social services assessments, ensuring effective communication with residents and their families.
  • Assist residents with financial and legal inquiries, including pension applications and referrals to legal and funeral services.
  • Facilitate discharge planning, including placement on waiting lists for community living, arranging home care services, and coordinating transfers to other facilities.
  • Provide or coordinate necessary counseling services and psychiatric medication management as required.
  • Ensure accurate and timely documentation in MDS care plans.

Qualifications:

  • Current social work licensure in the state.
  • Bachelor's degree in social work is required; a master's degree is preferred.
  • Preferred: One year of supervised social work experience in a healthcare environment.
