Registered Nurse Case Manager

The Clinical Case Manager is tasked with ensuring that patient care is effectively coordinated and managed, guaranteeing that services are delivered efficiently while overseeing clinical staff. Compliance with established professional standards and principles is essential.
Key Responsibilities:
- Receive and assess case referrals, reviewing pertinent patient information, including conducting home visits to evaluate hospice and/or home care requirements.
- Designate suitable hospice personnel to each case as necessary.
- Engage with the Medical Director regarding any inquiries about an individual's service eligibility.
- Analyze and assess each case through various methods, including home visits, meetings, record evaluations, and clinician services.
- Collaborate with clinicians to enhance performance and ensure the delivery of high-quality home care services.
- Examine patients' medical diagnoses, prognoses, medications, procedures, and clinical progress.
- Support clinicians in developing personalized care plans.
- Participate in case conference meetings with hospice staff to promote care coordination and interdisciplinary collaboration.
- Exhibit a willingness to assist colleagues.
- Conduct monthly record assessments and relay findings and recommendations to the Patient Care Coordinator and hospice staff.
- Support the Patient Care Coordinator and supervisory hospice staff in planning, executing, and evaluating educational programs.
- Engage in compassionate communication with patients and their families.
- Assist in the onboarding of new staff members.
- Promote community education and awareness.
- Be available during standard working hours, with potential additional hours to fulfill business responsibilities.
- Participate in public relations and community initiatives that enhance the organization's reputation within the hospice and healthcare sectors.
- Ensure accurate electronic documentation.
- Perform additional duties as assigned by management.
- Maintain records of nursing utilization and related activities for quality assurance and program development.
Education:
- Associate's Degree in Nursing (ADN) is required.
- Bachelor's Degree in Nursing (BSN) is preferred.
- Prior experience in hospice or a related healthcare environment is preferred.
- Current South Carolina (or compact state) RN licensure in good standing is required.
- Basic Life Support (BLS) certification is required.
- Certification as a Hospice and Palliative Care Administrator in good standing is required.
- Comprehension of hospice philosophy and the complexities surrounding death and dying.
- A strong commitment to core values such as excellence, compassion, teamwork, and integrity.
- Ability to collaborate effectively with nursing colleagues, physicians, and department leaders.
- Knowledge of pain and symptom management principles.
- Must possess a valid driver's license and have access to a reliable, insured vehicle.
- Exceptional organizational and computer skills.
- Strong verbal communication abilities.
- Flexibility to work varied hours and travel between locations as needed.
