Job SummaryWe are seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. In this role, you will be responsible for providing clinical care and support to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities- Conduct comprehensive patient assessments and develop individualized care plans
- Collaborate with the care team to ensure seamless care coordination
- Provide education and support to patients and families to promote health and independence
- Supervise and mentor LPNs and HHAs
- Document patient progress and outcomes in a timely and accurate manner
- Current RN license, specific to the state(s) you are assigned to work
- One (1+) year of clinical experience as a Registered Nurse
- Current CPR certification
- Valid driver's license, reliable transportation, and liability insurance
