About the Role:

Oak Street Health is seeking a highly skilled Nurse Practitioner to join our team. As a Nurse Practitioner at Oak Street Health, you will play a critical role in providing exceptional primary care to our patients.

Key Responsibilities:

  • Provide routine and acute care to patients
  • Conduct office visits and administer wellness visits and health risk assessments
  • Care coordination with other providers and specialists
  • Population health leadership and education of patients on their health conditions and care plans

Requirements:

  • Master of Science in Nursing or Doctor of Nursing Practice degree
  • National certification in a relevant specialty
  • Active, non-probationary state Nurse Practitioner license
  • Active DEA license
